Rapid Rehousing Case Manager
(STEHP Grant)
Full-Time: 37.5 hours per week in person
Annual Salary: $39,000 plus benefits
Full Job Description:
The Rapid Rehousing Case Manager will assist individuals and families experiencing homelessness in securing and maintaining stable housing. This role involves providing comprehensive case management services, including housing search assistance, landlord mediation, and connecting clients to community resources.
Position Responsibilities:
Conduct intake assessments to determine eligibility for rapid rehousing services.
Develop individualized housing plans with clients, focusing on overcoming barriers to housing stability.
Assist clients in locating and securing affordable housing, including negotiating lease terms and mediating with landlords.
Provide ongoing case management to support clients in maintaining housing, increasing income, and accessing necessary services.
Conduct home visits to ensure housing stability and address any issues that arise.
Collaborate with local service providers, landlords, and community organizations to coordinate services and resources for clients.
Maintain accurate and up-to-date client records in accordance with program guidelines.
Participate in community meetings and training sessions related to housing and homelessness services.
Other Duties as required
Qualifications:
Bachelor’s degree in social work, human services, or a related field preferred.
Minimum of 2 years of experience in case management, preferably with homeless populations.
Knowledge of local housing resources and services in Cayuga County, NY.
Strong organizational and communication skills.
Ability to work independently and as part of a team.
Proficiency in using case management software and maintaining detailed records.
Valid driver’s license and reliable transportation.
Preferred Skills:
Experience with housing first models and rapid rehousing programs.
Familiarity with HUD guidelines and local housing policies.
Bilingual (Spanish) is a plus.
Working Conditions:
Office environment with regular travel for home visits and community meetings.
