Intensive Care Manager - Transitional Housing
Full Time, 37.5 hours per week
Annual Salary: $39,000 plus benefits
Responsible for case management activities of CCHN’s Transitional Housing Program. This position will work to ensure that all vulnerable people in this target population have their housing, medical, behavioral health, environmental health and other identified needs addressed.
