Housing & Harm Reduction Case Manager
Full-Time: 37.5 hours per week in person
Annual Salary: $39,000 plus benefits
Full Job Description:
The HOPE Housing & Harm Reduction Case Manager provides supportive services for a caseload of 15 individuals who are actively using opiates and other drugs. Provide referrals to area resources and Narcan Training. Responsibilities include distribution of harm reduction supplies and materials, education of safe use and safer sex practices, referrals to community resources, documenting participant encounters, monitoring harm reduction supplies and ordering supplies when needed.
Position Responsibilities:
Proficiency with Office 365 (Outlook, Word, Excel, SharePoint, PowerPoint), Zoom, Facebook, Instagram
Excellent written and oral communication skills
Understanding and/or Lived Experience of opiate and other drug use
Understanding of Harm Reduction theories and practices
Work collaboratively with other staff
Ability to work collaboratively with community stakeholders
Attend trainings and maintain skills
Clean Driving Record
Reliable Transportation, Personal vehicle
Ability to work Independently and in a Team
Experience with data collection/data analysis
Lift at least 30 pounds
Eligible to legally work in the USA
Other duties as assigned
Job Requirements:
Bachelor’s degree in Public Health and one-year related experience or Associates degree and two years related experience or three years’ experience including lived experience (must be at least two years clean).
Working with opiate and other drug users
Working in community settings
