Our History

Cayuga Community Health Network (CCHN) is a collaborative group of community leaders and health and human service providers working together to improve the health and well-being of Cayuga County residents.

Founded in 1997, CCHN is a 501(c)(3) nonprofit organization and one of 37 Rural Health Networks established through New York State’s Rural Health Network Demonstration Program. CCHN began when the Cayuga County Health Department, Auburn Community Hospital, Cayuga Bank, East-Hill Family Medical, and the Cayuga County Medical Society came together to address shared concerns about the potential implementation of state-mandated Medicaid managed care.

That same year, the Health Network Steering Committee secured a grant through the New York State Department of Health’s Rural Health Network Development Program to develop a coordinated, Cayuga County-specific plan for health care providers as an alternative to a state-mandated system. This initiative laid the foundation for what was originally known as the Cayuga County Rural Health Network and established CCHN’s enduring commitment to collaboration and community-driven solutions that improve health across the county.

Our Goals

Cayuga Community Health Network (CCHN) works to strengthen the health and well-being of individuals, families, and communities throughout Cayuga County. We advance this mission by:

  • Fostering collaboration: Bringing together health and human service providers, community and business leaders, and residents to strengthen partnerships and improve coordination of care and services.

  • Identifying community needs: Assessing local health and social service needs, developing strategies to address gaps, and implementing initiatives that improve access to care and resources.

  • Advocating for resources: Building relationships with local, state, and federal government agencies to expand funding opportunities and support initiatives that address the health needs of Cayuga County residents.

  • Promoting health and wellness: Providing and supporting community-based programs, education, and prevention initiatives that address social determinants of health.

  • Offering Direct Services: Implementing programs that address housing, substance use, healthcare and service access, maternal health, health and wellness, and chronic disease prevention.
