Anna is the Executive Director of Cayuga Community Health Network, where she leads the organization’s programs, funding, and operations. With a strong background in nonprofit leadership and change management, she is passionate about building partnerships, streamlining operations, and creating organizational stability so programs can grow and have greater impact in the community.
Having lived in rural communities across the country, Anna understands both the beauty and the challenges of small-town life. That experience has shaped her deep commitment to ensuring everyone has the opportunity for optimal health outcomes, regardless of circumstance. She believes rural health networks are essential for coordinating care, strengthening local systems, and helping people connect to the resources they need.
Before joining the rural health field, Anna worked as an educator and school administrator, teaching in classrooms, directing educational programs and teams, and guiding outdoor education programs in Grand Teton National Park. She holds a BS in Conservation Biology and an MA in Education and currently serves on the Board of Directors for the New York State Association for Rural Health.
Outside of work, Anna loves spending time in the woods with her family and dogs, especially if it involves mountain biking, running, or skiing.
Holly Beauchine has been with Cayuga Community Health Network since 2017 and currently serves as a Social Care Network Community Health Worker, connecting individuals and families to critical services that improve health, stability, and quality of life.
Her work focuses on addressing social determinants of health through resource navigation, community outreach, advocacy, care coordination, and ongoing participant support. Holly is known for building trusted relationships, helping individuals overcome barriers and improve overall well-being.
In addition to her current role, Holly’s background includes work in chronic disease prevention, healthy lifestyle, fall prevention, family wellness, maternal and child health, healthy homes initiatives, including prior experience with the National Diabetes Prevention Program (NDPP) and Chronic Disease Self-Management programming.
Holly earned her Bachelor of Arts degree from Wells College with concentration in Sociology/Anthropology and Elementary Education. She brings years of experience, compassion, and strong commitment to improving lives across the region.
Alexa joined Cayuga Community Health Network in July of 2024 as an Intensive Case Manager under the HOPE grant. Alexa graduated from Cayuga Community College, earning an Associate Degree in Human Services in the spring of 2023. While she was studying for her Associates Degree, Alexa began volunteering for Chapel House which lead into a four year career serving Cayuga Counties homeless population through case managment. During her time at Chapel House, she was successful in a variety of roles including; Shelter Aide, Case Manager, and Program Supervisor. Her experience gives her a unique perspective of the needs and services for serving the vulnerable populations of Cayuga County. Alexa volunteered at The Salvation Army Family Shelter in Syracuse from 2016-2018 to assist children with their homework. When Alexa is not working she enjoys spending time with her rescue dog, Arrow, cheering on the Buffalo Bills at games, and spending time with her family.
Elizabeth joined the Cayuga Community Health Network in October of 2024 as the HOPE Grant Street Outreach Peer. She was raised in Auburn, N.Y. but spent 5 years in California before settling back here.
Always having had an interest and passion for helping people and the mental health field; and bolstered by her past-lived experience with Substance Use Disorder; Elizabeth entered into the Substance Use Disorder and Mental Health fields while in California as a Behavioral Health Technician.
She continued in those fields when she moved back to New York, working for Crouse until she began her work here at Cayuga Community Health Network in the Peer Position.
Elizabeth is a master “thrifter” and enjoys spending time with her son.
Cherrelle is a lifelong resident of Auburn, NY and a mother of four children: two girls and two boys. Cherrelle joined the Cayuga Community Health Network team as a Community Health Worker (CHW) in the PICHC program in December of 2024. She also continues to maintain her Doula business to work with expectant mothers and their families.
In 2012 Cherrelle earned her certification as a Medical Assistant through an accelerated program at Cayuga Community College. Through the program Cherrelle spent about 96 hours in the community shadowing EMTs, physical therapists, oncologists, MDs and nurses. Upon completion, Cherrelle entered the profession as an Optometrist Assistant from 2012 to 2024. There she assisted the doctor in preliminary screening and patient education. At the start of 2016 Cherrelle decided to expand her work experience and took on dermatology.
In March of 2020, Cherrelle completed Doula training provided by the Cayuga County Health Network. She also attended and completed breastfeeding training for Doulas. Having breastfed all four of her children, this training gave Cherrelle further knowledge and tools to support families with their breastfeeding journey.
Since becoming a doula, she has supported dozens of families in her community. As a Doula and Childbirth Educator, Cherrelle provides high quality care to guide women through one of the most transformative times of their lives. Having gone through 4 of her own pregnancies, Cherrelle provides women and their families with a sense of security and understanding for what they are experiencing. Cherrelle is a certified BRM pro and holds a certification with a Lamaze-Accredited Childbirth Educator Program.
Meghan joined the CCHN team in August of 2023, working part-time as a programming specialist due to a shared mission improving Rural Health. Her role has evolved as she became the Chronic Disease Prevention and Wellness Program Lead at Cayuga Community Health Network in 2024. Currently, She leads the development and implementation of community-based prevention initiatives focused on colorectal cancer screening, nutrition, and health behavior change.
She partners with community organizations, healthcare providers, and local stakeholders to design and deliver evidence-informed programming that improves engagement in preventive care.
As a Certified Lifestyle Change Coach and Licensed Food for Life Instructor, Meghan brings a compassionate, practical approach to health education, blending evidence-based strategies with real-life application. She is known for creating supportive, accessible environments where individuals feel empowered to make lasting changes.
Meghan holds a bachelor’s degree in Psychology and is currently pursuing her Master of Public Health. She is CITI Program certified and contributes to academic research through literature reviews and public health scholarship, with a focus on prevention and community-based interventions.
In addition to her role at Cayuga Community Health Network, Meghan is the owner of Synchronistic Elements, where she provides consulting services in wellness programming, behavior change, and community-based health initiatives, and contributes to scientific research and evaluation.
Outside of her professional work, Meghan is a mother of four and enjoys photography, writing, music, and spending time outdoors with her family.
