Welcome to the Cayuga Community Health Network, Inc. website!

 The Cayuga Community Health Network, Inc. is a 501( c) (3) nonprofit organization, established in 1997, we are one of thirty-seven Rural Health Networks’ in New York State.

“The Rural Health Networks in the Finger Lakes Region of NYS serve 10 counties in southern and western New York. Rural Health Networks take the initiative to make the health care system work better in rural areas. A Rural Health Network can be defined as “a formal organizational arrangement among rural health care providers (and possibly insurers and social service providers) that uses the resources of more than one existing organization and specifies the objectives and methods by which various collaborative functions are achieved.”

The Cayuga Community Health Network conducts the following activities/goals/initiatives:

    • the Network is the lead agency of the Tobacco Free Partnership- Cayuga County. Funded by the NYSDOH, the tobacco free partnership works collaboratively with local and regional partners on tobacco control issues. The partnership strives to work towards changing the social norms, implementing smoke-free policies and behavior change while working with local health care providers (and the cessation center).     • the Network is the lead agency for the CNY Diabetes Prevention Partnership Program for Cayuga County. Funded by a grant from the NYSDOH, the Diabetes Prevention Program serves the counties of Cortland, Cayuga, Madison, Onondaga and Tompkins. The partnership works towards education and prevention of diabetes within this region and works collaboratively with local community organizations to promote awareness of diabetes and its prevalence within our society.     • the Network works to increase community awareness and knowledge of healthcare and accessibility issues through ongoing multi-media health promotion and social marketing campaigns. This initiative works to empower residents to overcome barriers to healthy behaviors and access, and seek opportunities for achieving optimum health.

    • The Network works towards implementing change in the areas of health promotion programs and with community providers to employ more evidence based best practices.

    • the Network works collaboratively with locally-based partnerships to address local health care system gaps in Cayuga County and disseminate information on innovative approaches to the delivery of health care.
