Diabetes Educational Program
CCCHN was formerly the lead agency for a grant from the New York State Department of Health called the Central New York Diabetes Prevention Partnership which served the counties of Cortland, Cayuga, Madison, Onondaga and Tompkins. The Diabetes Program was cut from funding in 2009 due to budget cuts.
However, CCCHN continues to implement this program through donations, sponsorships and fundraising. The Network strives to educate the public of diabetes risk factors and prevention and assist those diagnosed with resources and information to minimize complications from their disease.
CCCHN provides the following to the community at no cost:
- “Lunch & Learn” Series
- Nutritional Education Program
- Cooking Classes
- Support Group
- Various other Workshops and Seminars
Diabetes Type 2, referred to as a silent epidemic in our culture, affirms our goal for this program. That is, to increase awareness of specific behaviors we can adopt to improve our community’s health, starting with each individual within our community.
Diabetes Support Group
Group facilitator: Michelle Dart, RN, MSN, CDE.
The Diabetes Support Group meets on the third Monday of each month at Auburn Memorial Hospital from 6:30 – 7:30 pm, 3rd Floor Conference Room. Parking is Free. Public is welcome.
